We are excited to begin the use of online waiver collection through MySAM. The next time you login to MySAM you will be prompted to read and electronically sign and submit the annual Waiver and Release of Liability Form. If you have already signed a paper copy, you will still need to submit an electronic copy on MySAM for 2009.

Please login to MySAM before your next game to complete the Waiver as it will save time and paper by not having to complete a paper waiver in person before your next game. The online waiver will automatically display once you login to your MySAM account and it only takes seconds to complete.

Use of the online waiver will also allow new players to create their own MySAM account, and be added to a roster immediately when the team captain utilizes the “Invite Someone” feature in MySAM, which is something that quite a few of you have asked for. The team captain will no longer have to contact us to have someone added. If you are the parent of a child that also participates with us, you will also sign the waiver for each child associated with your account.
